The Systems and Structures That Power Primary Health Care in Nigeria

What really makes Primary Health Care work? It is more than a building, health workers, medicines, or equipment. Behind every functioning PHC facility is a complex system of laws, policies, financing, institutions, health workers, supply chains, information systems, referral networks, regulators, and communities.

This article takes a closer look at the “Rules, Actors and Machinery” that determine whether Nigeria’s PHC system actually delivers quality care. It explores who is responsible, how resources move to the point of care, why coordination matters, and what happens when critical connections in the system fail.

A key message is that strong institutions alone do not guarantee a strong health system. A facility may have a building but no health worker, medicines but no financing, or skilled staff but a broken referral system. The real challenge is making all these components work together around the needs of the patient.

The article also asks some difficult questions: Who is accountable when PHC fails? Are our facilities truly functional? Does investment translate into better care? And are we measuring what matters to patients?

Ultimately, Nigeria needs to move from structures to functionality, mandates to accountability, institutions to integration, and programmes to a truly connected health system. When the machinery works, people experience reliable care and over time, that reliability builds public trust.
