From Building Blocks to Transformation: A Strategic Agenda for a Primary Health Care Renaissance in Nigeria
Nigeria’s Primary Health Care system has invested significantly in facilities, health workers, financing, medicines, technology, and information systems. Yet, many communities still experience unreliable services, stock-outs, staff shortages, long waiting times, weak referrals, high costs, and declining trust.
This article argues that having the building blocks is not enough. The real challenge is making them work together as one integrated, people-centred and accountable system.
The proposed Primary Health Care Renaissance calls for six major shifts:
- Administration → Accountable Governance
- Fragmented Financing → Strategic Purchasing
- Workforce Numbers → Workforce Performance
- Facility-Based Care → Integrated, People-Centred Care
- Data Collection → Intelligence for Action
- Commodity Availability → Resilient Supply Systems
At the heart of the agenda is a simple but powerful idea: PHC transformation happens when governance, financing, health workers, medicines, information, service delivery and communities work together. The ultimate measure should not be how many facilities are built or how much money is spent, but whether people receive accessible, affordable, equitable, quality and respectful care.
The article also explores why trust, equity, community participation, effective coverage, local leadership and performance management must become central to the next generation of PHC reform. It presents a practical five-point agenda for making facilities functional, aligning financing, empowering frontline managers, strengthening performance management, and making communities genuine partners in healthcare.
